Church Forms

Saint John Baptist Church has official forms that should be used when accessing services or resources. These forms can be filled out online and printed. You may print the forms with your completed information and bring them or mail them to the Church Office. Please make sure that you check with the Office Manager or Secretary before completing the form to ensure that your requested date for service or resource is available. If you reserve the facility, you must submit your completed form and we must receive the same within forty- eight (48) hours. If the completed form is not received, the date that you have reserved by telephone will be removed.
